Advance Directives: The National Hospice and Palliative Care Organization sponsors the website caringinfo.org (linked here) that offers downloadable advance directive forms that are specific to each state’s requirements. (The site does not offer legal advice on how to complete the form. Legal questions should be directed to attorney with expertise in this area.)

Spirituality and Medicine: For physicians and other health professionals, this page from the University of Washington’s Medical School’s Ethics on Medicine website offers information on how to take a spiritual history, how health professionals can work with hospital chaplains, and what role your personal beliefs should play in the physician-patient relationship.
